If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. Travel Advice

Prior to your Travel, we would like to you to complete a Travel Assessment Form so that we can get a better understanding of where you are travelling, how long for and what activities you have planned.  

This will avoid unnecessary appointments. It also means we can research the appropriate travel information before your holiday.  Please note we will only be able to give the following travel vaccinations:-

Tetanus, Diphtheria, Polio, Typhoid and Hepatitis A

All other travel vaccinations will need to be given by a Travel Clinic e.g. Rabies, Hepatitis B etc.  Please see details below for local Travel Clinics.

Please complete and return the Travel Assessment Form at least 7 weeks prior to your travel date so we can safely vaccinate if needed.  If you are travelling in less than 7 weeks, we recommend that you seek advice from a travel clinic (details below) to ensure you are safely vaccinated before your trip.

Travel Forms will be available to collect from the Practice reception.  Please can you complete the Travel Assessment Form and hand them back as soon as possible to the Reception desk so that we can organise an appointment with the nurse as required.

If you are up to date with all your necessary vaccinations and a travel appointment is not necessary, we will contact you by phone to let you know. If an appointment is required, you will be called to organise a suitable appointment with a nurse.
